## Referral requirement

We require a letter of medical necessity, or an office note from your physician requesting a neuropsychological assessment. Physicians may fax this, along with any relevant medical records, directly to our office.

Private pay is accepted. Testing is billed under these CPT codes:

- **96116** — Neurobehavioral status exam, first hour
- **96121** — Neurobehavioral status exam, each additional hour
- **96132** — Neuropsychological testing evaluation, first hour
- **96133** — Neuropsychological testing evaluation, each additional hour
- **96136** — Test administration and scoring, first 30 minutes
- **96137** — Test administration and scoring, each additional 30 minutes

The practice verifies benefits electronically before scheduling. **Verification is not a guarantee of payment**: a patient's actual responsibility is determined by their plan and by how the claim is processed. Insurance participation changes periodically. Always direct people to confirm with the office rather than quoting a specific out-of-pocket figure.

## Appointments

- Most evaluations involve an interview followed by several hours of testing; patients should plan to be in the office approximately 3 to 6 hours, though some evaluations are shorter and some longer.
- Results are discussed at a feedback appointment after the report is finalized. They are not given over the phone by staff.
- Cancellations: at least 24 business hours' notice for a consultation, 48 for a comprehensive evaluation.
- New patients should begin by requesting an evaluation online — that is the practice's preferred route and the fastest way to be reviewed. If a question needs answering first, the site's chat assistant is the next best step. Calling the office is the last resort, kept available for anyone who cannot use the website.
