We are dedicated to providing high-quality, individualized neuropsychological evaluations that clarify cognitive strengths and difficulties and provide practical recommendations for next steps.
Our practice focuses on neuropsychological evaluation. We do not provide counseling, psychotherapy, cognitive rehabilitation, neurological consultation, or psychiatric assessment. Request an evaluation
Tell us who the evaluation is for, why it has been recommended, and how to reach you. It takes a few minutes, and you can stop and resume at any point.

Have your doctor send us medical documentation
We need a letter of medical necessity or an office note from your physician explaining why a neuropsychological evaluation is needed. Please ask your physician to fax it directly to our office at (631) 366-2043. We cannot begin the insurance review or scheduling process until we receive it.

We verify your coverage
Submit your insurance information online, and we can electronically check the benefits your health plan defines for our services.

Intake paperwork
For your convenience, intake forms can be sent by secure electronic link so they can be completed before your appointment. If you prefer, paper forms can be completed at your consultation.

Your evaluation
The evaluation begins with a clinical interview with Dr. Barnoski to discuss the reason for the referral, relevant history, and current concerns. When appropriate, the evaluation may also include standardized testing of areas such as memory, attention, learning, language, and problem-solving. The specific components and length of the evaluation are determined by the referral question and individual needs.

Findings and direction
When your evaluation report is complete, you can meet with Dr. Barnoski to review the findings, discuss any diagnoses, and go over recommendations and next steps.
